Membership Terms: Billing, Cancellations & Refunds

Monarch Med NP provides care through a membership model. By enrolling, you agree to the following billing, cancellation, and refund terms. (All timeframes are based on Pacific Time.)

1) Membership Billing

  • Membership fees are charged on the 1st of each month.

  • If you enroll mid-month, your first membership payment is due at the time of enrollment/booking, and then renews on the 1st of the following month.

  • Membership fees cover access to membership services as described on our website (appointments and portal messaging within the scope of your program). Labs, imaging, and medications are not included unless explicitly stated.

2) How to Cancel Membership

  • You may cancel your membership at any time before the next billing date to avoid being charged for the next month.

  • Cancellations take effect at the end of the current paid billing period unless otherwise stated in writing.

  • To cancel, submit your request through the patient portal or email Monarchmednp@gmail.com.

3) Membership Refund Policy

  • You may request a refund within 3 calendar days of a monthly membership charge only if no clinical services have been provided during that billing period.

    • “Clinical services” includes appointments (telehealth or in-person) and care activities such as treatment plan updates, lab interpretation, and medication/refill management.

  • No refunds are provided after 3 calendar days from the charge date, or once clinical services have been provided.

  • No prorated refunds for partial months.

4) Foundational Assessment (Initial Consultation) Terms

The Foundational Assessment is required to establish care and includes:

  • Initial visit (60–90 minutes)

  • One telehealth follow-up (30–45 minutes) for lab review and initial treatment plan creation

Fee: $185 — labs are not included.

Foundational Assessment Billing & Refunds

  • Payment is due at the time of booking.

  • The Foundational Assessment is non-refundable once completed.

  • If you cancel the Foundational Assessment within 24 hours of your scheduled appointment time, the fee is non-refundable.

5) Appointment Cancellation / No-Show Policy (All Visits)

  • Please cancel or reschedule at least 24 hours in advance.

  • Appointments canceled with less than 24 hours’ notice, or missed (“no-show”), may incur a $25 late-cancel/no-show fee.

6) Communication Expectations

  • Secure portal messaging is for non-urgent questions related to your care plan.

  • Message response time is typically within 24 business hours, Monday–Friday (excluding holidays).

  • Portal messaging is not monitored 24/7 and is not for emergencies. If you believe you are experiencing an emergency, call 911 or go to the nearest emergency department.
