1) Website Purpose and No Medical Advice
This Website is provided for general informational purposes only and is not medical advice. Content on this Website is not a substitute for professional medical evaluation, diagnosis, or treatment. Do not ignore or delay seeking medical care because of something you read on this Website.
If you think you may have a medical emergency, call 911 or go to the nearest emergency department.
2) No Patient Relationship Through the Website
Using this Website, sending a message through a Website form, or signing up for a newsletter does not create a provider–patient relationship. A provider–patient relationship is established only through a scheduled clinical visit and completion of required intake/consent processes.
3) Do Not Send Private Health Information (PHI) Through Website Forms
Website forms and email may not be secure methods of communication. Do not submit sensitive medical or personal health information through Website forms. For clinical questions, scheduling, membership enrollment, or secure communication, use the patient portal/EHR.
4) Patient Portal / Third-Party Services (OptiMantra)
Scheduling, membership enrollment, payments, and portal communications may be provided through a third-party electronic health record (“EHR”) / patient portal, such as OptiMantra. When you click links to the portal, you are leaving our Website and are subject to the portal provider’s terms and privacy practices. We are not responsible for third-party websites or services.
5) Telehealth Location Requirement
Telehealth services (if offered) require that you are physically located in the State of Nevada at the time of the appointment. We do not provide telehealth services to patients located outside Nevada at this time.
6) Medicare
Monarch Med NP is not accepting Medicare patients at this time.
7) Membership and Services
We may offer membership-based care and/or specific services (such as hormone therapy and medical weight management). Any descriptions of services, pricing, or inclusions on this Website are for informational purposes and may change. Final terms, consents, and applicable policies are provided during the onboarding and enrollment process and/or within the patient portal.
Foundational Assessment: We may require an initial appointment (“Foundational Assessment”) before starting ongoing care. Fees, what is included, and what is excluded (including lab costs) will be disclosed before you book.
8) Billing, Cancellations, and Refunds
Billing terms (including membership fees, payment timing, cancellation, and refund policies) will be provided during booking/enrollment and/or inside the patient portal. If you have questions, contact us using the information below.
9) Messaging and Response Times
If secure portal messaging is available, it is intended for non-urgent communication related to your care plan. Our typical response time is within 48 business hours (Monday–Thursday; excluding holidays). Messaging is not monitored 24/7 and is not for emergencies.
10) Intellectual Property
Unless otherwise stated, all content on this Website—including text, logos, graphics, images, and design—is owned by or licensed to Up River Wellness (DBA Monarch Med NP) and is protected by intellectual property laws. You may view, download, and print content for personal, non-commercial use only. You may not reproduce, distribute, modify, or create derivative works without our written permission.
